Tables below present, for each year, the list of spacecrafts that are considered failures. We consider five types of failure:
Pad failure occured before the launch, generally a catastrophic destruction of the launcher and/or its payload(s).
Launch failure occured when the launcher failed to place its payload(s) into Earth orbit.
Orbital failure occured when the payload(s) failed to reach its proper orbit (usually, the upper-stage of the launcher failed to reignite or the apogee-kick motor of the spacecraft failed to ignite).
Mission failure occured when it is considered that the mission was not accomplished (for example, a spacecraft that remains "silent" after reaching orbit, or a probe that crashs on (or past by) its intended target). (We do not count partial mission failures and some mission failures are probably unknown.)
End-of-mission failure occured when, generally, the payload was not recovered as intended (it burned in the atmosphere or is destroyed upon landing).
Note that we do not count partial failures, spacecrafts that had not met all their mission objectives. We will evnetually include those partial failures in the lists below, but not counted them as "failures".
