CMS-0057 is just medical prior authorization. There's actually a separate pending rule for pharmacy prior authorization with NCPDP in CMS-0062 (reginfo.gov/public/do/e…)
The CMS rules only cover CMS plans and don't guarantee employer based plans benefit from electronic prior auth
