A HR of 185 is not interpreted in isolation.

In the operating room, the question is not only:

“What arrhythmia is it?”

The truly useful question is:

What is the intraoperative period doing to the heart to make it electrically unstable?

Because that tachycardia can be sustained by hypoxemia, hypercapnia, catecholamines, pain, superficial anesthetic plane, hypovolemia, ischemia, reperfusion, electrolytes, temperature, drugs, or a specific surgical maneuver.

The monitor shows the rhythm.

But the mechanism explains the management.

And therein lies the difference between reacting… and treating with physiology.
