Your body has two fuel sources: glucose and fat.

Metabolic flexibility is the ability to switch between them smoothly, burning whichever is available. Chronically high insulin keeps you locked in glucose mode. You literally cannot access stored fat when insulin is elevated, regardless of how clean you eat.

This is why constant grazing undermines fat loss. Not the calories. The hormonal environment that never gets a break to reset.
